What to do before your visit
We want you to have a helpful and productive experience when coming to your appointment. In order for us to provide you with the best possible care, there are some things you can do before your visit.
An accurate medication list including doses and the number of times you take your medicine is as important as obtaining a history and physical exam. If you do not have a list, then you can bring in all of your pill bottles so that they can be reviewed with your health provider.
Let us know if you've had any recent cardiac hospitalizations, tests or procedures when you check in. If you've received care outside of the local area, please let the office know before your appointment so that these records can be reviewed before your visit. You may need to sign a medical release form in order for us to obtain these records from your hospital. You can download a PDF file of the medical release form here.
If you are a new patient to the practice, you can fill out a new patient intake form at home by downloading the PDF file here.
Remember that you should check with your insurance carrier to verify that we are in network providers with your plan. Please let us know of any changes involving your insurance as soon as possible. You will be required to pay your insurance co-payment when you check in for your appointment. Payments can be made by cash, check, debit or credit card.
